Referrals to Adventist Behavioral Health may be made by mental health professionals, schools, therapists, hospitals, social service and juvenile service personnel, clergy, managed care organizations, other inpatient treatment programs or residential treatment programs, and by individuals or families themselves.

What insurance does Adventist Behavioral Health accept?

Adventist Behavioral Health participates in nearly all area commercial insurance plans, HMO's, and managed care organizations. The hospital is a provider for Medicare, Champus and Maryland Medicaid (Maryland Health Partners).  Adventist Behavioral Health Residential Treatment Center at Rockville is a provider for Medicaid.  For those without insurance coverage, our business office can help the patient or family structure a payment schedule that meets an individual's financial circumstances.

To obtain a medical record, please contact our Medical Records Department at 301-217-5403. A release of information/consent form, signed by the patient and/or family, is required. 

What documents need to be included in the referral packet for Adventist Behavioral Health Residential Treatment Center at Rockville?

Please include the following documents when sending a referral packet for admission consideration:

• Psychiatric evaluation with five axis diagnosis
• Psychological testing including IQ score
• Educational assessments
• Current Individualized Education Plan (if applicable)
• Transcripts from last 2 academic years (if individual is classified as a regular education student)
• Psychosocial history or summary
• Copy of Birth Certificate, Social Security Card, & Immunization Record (showing two MMR immunizations)
• LCC and ARD meeting minutes
• Guardianship papers (if agency has limited guardianship, the orders MUST specifically state that the agency has the right to consent to medical, educational, psychiatric care & can consent for psychotropic medication)
• Court order or adjudication hearing order
• Information regarding current and previous placements
• Information about long-term placement goals after residential treatment
• Contact information of involved parents or relatives
• Sex offender evaluation (Adolescent Sex Offender Program only)
• Police report of offenses and victim's statement (Adolescent Sex Offender Program only)
